I killed the clone this evening solo in hardcore with a barb, after thinking
carefully if I wanted to risk my character. My setup was:

Laying of Hands
Stormshield
Guardian Angel (to max resists)
String of ears
Blackthorns Face (Prevent Monster Heal and lighting absorb)
Rising Sun Amulet (fire absorb)
Dwarf Star (fire absorb)
Raven Frost (can not be frozen and Dex)
Lightsaber (lightning absorb)
Nats boots (speed and lightning resist)

With charms, I had:
90 fire resist
89 lightning resist
70 cold resist
-3 poison resist

It took about 20 minutes, my merc died only once, and as long as I stood
right in front of him, Diablo ignored my merc for the most part. I had a
full belt of rejuv, but didnt really need them...I went to town probably 8
times to heal. Frankly, I was terrified, but it turned out to be a lot less
scary that I had imagined, given my setup.

The charm was +20 all resist, +19 all attributes, +5% experience, and +1 all
skills. I couldn't sleep for an hour after doing it.

Good question. He was a Maxed WW in 1.09, but I started to shift new points
into Concentrate, and I am very glad I did, since WW is not very useful in
Act 5 Hell. However, most of the damage came from Berserk, which I think I
had like 10 points in. I used Concentrate to leech back a little life and
mana, but his resits were so high that I couldn't hit him enough to leech
back everything I needed, so I had to go to town to heal and regenerate
mana.

Blackthorns Face (Prevent Monster Heal and lighting absorb)
The most important item in the setup.

Stopping his massive regen makes it much easier.
